To access the form edit history of deleted forms, click the top menu and navigate to Setup and Deleted forms on the right side menu of the page as shown in the screenshot below.
The page will show you not only which user has trashed the form + date, you can also access the edit history of the form by clicking History.
The Form history (deleted form) page has a Edit tab and Read tab showing their respective history.
